Surprisingly, there is very little out there about the specifics of analyzing performance problems.

Analyzing Performance Problems: or You Really Oughta Wanna by Robert F. Mager, Peter Pipe is the classic book on the subject. It's been around for years and proved its worth to a couple of generations of bosses.

Bringing Out the Best in People by Aubrey C. Daniels is specifically about performance management with an excellent section on consequences. This is an excellent review of the science that lies beneath the surface of people management. The author's style is not conducive to quick reading, though, so you may want to review some of the contents of the book before buying.

The best book I know on consequences, and one of the best management books ever is The Greatest Management Principle in the World by Michael Leboeuf. This book is sometimes hard to find, but it's worth picking up even a used copy.
